Medical Record Canvass

Our Medical Record Canvass is a time-saving service that provides insurances companies with the locations and dates that the subject received treatment through a healthcare provider. All contact information is provided, including: address, phone number, name of facility and name of the contact person.

With your release or subpoena, we can also obtain the full medical records from any of the facilities that your subject visited.

Not all injuries or illnesses can be researched on common insurance databases (INDEX, Accident, W/C). Therefore, the benefit of this service is locating current or preexisting conditions never reported to you. Other advantages are specific to each search. For example: someone claimed carpal tunnel syndrome and had received no hospitalization, but previously attended an occupational clinic for a full year. Another example is a pharmaceutical records search that would uncover someone addicted to pain medications. Each scenario can provide different benefits, depending on the subject’s claim and prior history.

Medical Records Canvass is a service that we provide, which allows insurance companies to know the dates and locations regarding when the subject went to the specified healthcare provider. Completed in 48 hours this service is HIPPA compliant.

Unmatched 48-Hour Turnaround

We guarantee an industry-best 48-hour turnaround, searching either 15 locations or within a 20-mile radius of the claimant’s residence.

Fast & Cost-Efficient

The cost of a canvass is a flat rate of $150.00 per each facility search (i.e. hospital, pharmacy, occupational health centers, clinic, or whichever institution you want to research). We offer a discount for ordering a comprehensive medical canvass, which includes a hospital, chiropractic and pharmaceutical facility for a flat rate of $400.00 for all three.

We can also pair the canvass with a social security number search that might yield additional addresses for a fee of $35.00 per search, or $15.00 if paired with a comprehensive canvass. Additional canvasses can be performed at the new addresses, potentially uncovering more treatment dates. Another option, if the initial search does not yield any records, is an expanded search, which is offered at a discounted rate of $125.00 per search or $300 for a comprehensive.
